FILLING IN FORMS by Samson Mwita

FILLING IN FORMS
A form is a sheet of paper with blank spaces in which specific information should be filled in. In recent years people are obliged to fill different forms for different purposes. We fill forms in the bank (for cash deposit of cash withdrawal), hospital forms, joining instruction forms,  loan application forms, company and business registration forms, combination and A-level school selection forms, etc. This shows that filling in forms is a subject worthy our time.
These forms can be filled in on hardcopy or online softcopy. In most cases the forms are filled in and submitted in hardcopy. However, due to the advancement in science and technology, and information and communication technology in particular, some forms can be filled in and submitted online. Sometimes these online forms have read-made options that you simply need to select and click ok. E.g. Online Loan Application System by (HESLB). Others can be filled in online, then printed and submitted in hardcopy. Whatever medium you are using there are things you must pay attention to:

Things to consider when filling in a form.
a)      Study/read the form carefully before filling it in. Read the instructions carefully before attempting to fill in the form. E.g.: you may be required to use Block/capital letters, black ink, write in words not numbers etc.
b)      Be neat and legible. Use a smart handwriting and avoid cancelling or rubbing.
c)      Be brief and accurate. Forms have limited spaces if you cancel you may not have enough spaces to write the required information.
d)      Follow instructions/steps carefully. Some forms are filled step by step; if you skip one step (especially for online forms) you may not be allowed to go to the next step.
e)      Use familiar symbols or abbreviations. Don’t use your own symbols and abbreviations. Use the ones that are common in that field. E.g. NMB, Tsh, Co. Ltd, HESLB, TCU, UDSM, UDOM etc
f)       Sign and supply the date.
